Domestic partner health insurance information now available

Detailed enrollment information for domestic partner health insurance is now available to UW–Madison faculty and staff.

Visit http://benefits.wisc.edu and view the document called “ETF Domestic Partner Benefits, Questions and Answers” under the News and Events section. The document, produced by the Wisconsin Department of Employee Trust Funds, is a complete guide to the process

To enroll, faculty and staff must file an Affidavit of Domestic Partnership form (ET-2371) with ETF, certifying that you meet the requirements under Chapter 40 of Wisconsin law. They must also fill out applications for the benefit programs in which they would like to enroll their domestic partner. Contact your payroll and benefits representative or visit the ETF Web site for applications.

For the 2009 It’s Your Choice enrollment period, submit the affidavit to ETF and the Group Health Insurance Application/Change form (ET-2301) to your employer at the same time, with a copy of the Affidavit attached to the insurance application. ETF will send an acknowledgement of your Affidavit establishing the effective date of your Chapter 40 domestic partnership.

On Sept. 21, several hundred members of the community, including Chancellor Biddy Martin, Rep. Mark Pocan and Vice Provost for Faculty and Staff Steve Stern, attended a celebration and question-and-answer session with representatives of the Office of Human Resources.

UW–Madison spent nearly a decade lobbying for domestic partner health insurance as an issue of equity, recruitment and retention.
