Flu shot availability begins Nov. 3
The Environment, Health and Safety Department will again offer a variety of times and locations for on-campus flu shot clinics.
Flu shot clinics will be provided at 12 campus locations beginningMonday, Nov. 3, and running through Friday, Nov. 14. UW–Madison hasset a goal of immunizing 5,000 faculty, academic and classified staff, says Jim Morrison, occupational health officer.
For employees with Dean Care, Physicians Plus, Group HealthCooperative or Unity health insurance, flu shots are administered atno charge if you bring your insurance card with you to the clinic.
If you do not bring your insurance card with you, a fee of $28 forthe flu shot will be required. If you pay for the shot on site, areceipt will be provided so you can obtain reimbursement from yourhealth insurance provider. Please plan to pay with cash or check.
Clinics are designed specifically to accommodate demand fromemployees, not spouses or children, who are encouraged to receiveshots from a health-care provider.
Faculty and staff sites are as follows. All clinics are 9 a.m.–3 p.m. unless noted.

- Nov. 4 (Election Day): Clinics will be held at polling stations across Dane County
- Nov. 5: Waisman Center, Room 120–122
- Nov. 5: Physical Sciences Laboratory (PSL), 3731 Schneider Drive, Stoughton, PSL Large ConferenceRoom, 3–5p.m.
- Nov. 6: Union South, TITU
- Nov. 7: Ingraham Hall, Room 103
- Nov. 10: Medical Sciences Center, Room 2010
- Nov. 11: Armory and Gymnasium, TITU
- Nov. 12: State Hygiene Lab, Fish Bowl, 9 a.m.-noon
- Nov. 12: WISPIC, 6001 Research Park, noon-3 p.m.
- Nov. 12 (midnight clinic): Veterinary Medicine Building, 10:30 p.m.–1:30 a.m.
- Nov. 13: Wisconsin Alumni Research Foundation, Room 132
- Nov. 14: Grainger Hall, Room 3190
